Upon completion of a NDC, students must submit
1) an official transcript 
Once a course is complete, students must request that it be added to their transcript via the application below.
Transcripts for courses taken outside of SRVUSD to be placed onto the student's SRVUSD transcript are due to the student's high school counseling office as follows:
  • By Friday, August 12, 2022: students wishing to use a NDC for advancement purposes. Please plan for shipping time: all transcripts should be ordered from the outside institution by August 3, 2022 to ensure delivery by this deadline.
  • By Tuesday, September 13, 2022; students wishing to put a Summer, 2022 NDC onto their SRVUSD transcript
  • By Friday, January 20, 2023: students wishing to put a Fall, 2022 NDC onto their SRVUSD transcript
  • By Friday, May 19, 2023: students wishing to put a Spring, 2023 NDC onto their SRVUSD transcript
  • For current eighth-grade students wishing to put a NDC on their high school transcript, the course must have been completed after June 6, 2022.  
  • Please complete the form in the link above and attach it to your transcript.
You can mail or drop off documents at the MV Attendance Office window. The NDC form can be emailed.
Official transcripts can be mailed to my attention at the address above or emailed to me directly from the originating school. 
Alternately, If the institution offers an email option to send it directly to your school, this is acceptable and will be considered an official copy (email to

Please email your request to Sarah Sirota  (, Monte Vista Registrar.   The request should include written authorization to release the information from the applicant or it will not be processed.


Students transferring from or leaving Monte Vista High School to attend another school must follow the withdrawal procedures. This procedure is initiated by the Registrar and includes completing a Withdrawal Form and returning all library and text books.

Immunization records and final transcripts will not be released until a completed Withdrawal Form is returned to the Registrar.
